Annual Test Report Submission - ONLINE ONLY
As of 2017, Tualatin Valley Water District (TVWD) no longer accepts paper test reports. Registered State certified testers must electronically submit their test reports via our online FormLink Systems portal at http://palladium.formlinksystems.com.
Test reports are required to be submitted within 10 business days of performing the test. Testers must submit test reports for new assemblies, annual tests, failed tests, and for assemblies which were removed/replaced. If a tester is submitting a report for a newly installed assembly, please email the test report to backflow [at] tvwd.org (backflow[at]tvwd[dot]org) so the new assembly can be added in FormLink Systems.
Residential customers must have passing test reports on file by September 1st each year.
Setup a FormLink Systems Account
To setup your FormLink Systems account for test report submissions, please email backflow [at] tvwd.org (backflow[at]tvwd[dot]org) with the following information:
- company name
- company email
- company phone number
- full company address
- copies of OHA tester certification(s) and gauge calibration(s)
Update Your State Certification and/or Gauge Calibration
If you are able to log into FormLink, but unable to enter a test report or unable to select your gauge, you may need to update your certification(s) or gauge calibration(s). Email a valid copy of your Oregon Health Authority Backflow Assembly Tester certification and/or your annual gauge calibration certificate(s) to backflow [at] tvwd.org (backflow[at]tvwd[dot]org).
