Formlink - New Location Codes as of July 4th, 2022

Due to our Billing System change, location codes have changed. You may still access backflow assembly information and enter test report data via the assembly's serial number. Email us at backflow@tvwd.org if you need additional assistance/information.

Annual Test Report Submission - ONLINE ONLY

As of 2017, Tualatin Valley Water District (TVWD) no longer accepts paper test reports.  Registered State certified testers should electronically submit their test reports via our online FormLink Systems portal at http://palladium.formlinksystems.com.

Test reports are required to be submitted within 10 business days of performing the test.  Testers should submit test reports for new assemblies, annual tests, failed tests, and also for assemblies which were removed/replaced.

Update Your State Certification and/or Gauge Calibration

If you are able to log onto FormLink but unable to enter a test report or unable to select your gauge, you may need to update your certification(s).  Email a valid copy of your Oregon Health Authority Backflow Assembly Tester certification and/or your annual gauge calibration certificate(s) to backflow@tvwd.org
